Within-facility Transfer Navigator 

The Transfer Navigator is used when patients move from one level of care (e.g., acute) to another (e.g., critical care) within the same facility. All activities needed for a safe and complete transfer can be accessed from the Transfer Navigator, both simplifying prescriber work and ensuring that orders, documentation and tasks are coordinated. 

The most important steps, in order, include the following:

• Clean up current orders

• Review medication reconciliation

• Place transfer orders

• Prepare a transfer summary
