Community and Provider Notification Forms
Simple letter forms can be used to alert community health care organizations or providers to important information enabling the transfer or continuity of patient care. The cForms workflow keeps a record of the communication in the chart while using appropriately secured communication channels.
Notification forms are found the same way that other communication (letter) forms are revealed via the Sidebar, Consent & eForms Navigator or Communication Activity.
Pharmacy Notifications
Community pharmacies may benefit from listings of all current medications, recent medication discontinuation instructions, or other information about relevant therapeutics or adverse reactions.
An outpatient medication community pharmacy notification is available to support outpatient care, as well as situations where inpatient care requires "external" pharmacy services.
Patient Attachment Notifications and Requests
Patient attachment notifications and requests relate to assigned relationships between patients and members of their health care team. An early focus is on primary care attachments.
Primary Care Provider Attachment Confirmation
A Primary Care Provider (PCP) Attachment Notification (confirmation) letter alerts a patient's primary care provider that they have been identified in this role by the patient. The letter includes instructions for making corrections to the patient attachment part of the health record. A Primary Care Provider (PCP) Attachment Request seeks a potential primary care provider or service that a patient has identified them as a possible source of ongoing care. Optional chart content (e.g., patient care coordination note, consult note, etc.) can be selected to support the communication.

An "Finding a Family Doctor" information letter can be generated for printing and providing to patients who need help finding a source of primary care services.

Continuity of Care Notifications and Requests
A Continuity of Care letter can facilitate properly documented transfers of care between prescribers, when a full referral workflow is not appropriate or needed. Optional chart content (e.g., patient care coordination note, consult note, etc.) can be selected to support the communication.
Prescriber Notifications
Letters can be generated to facilitate communication about exceptional care plans, immunization records and chronic disease management plans.
