Connect Care Fall 2026 Upgrade

The Fall 2026 Upgrade to the Connect Care clinical information system will occur on November 19, with changes to most Connect Care applications. These changes are primarily enhancements and will affect all prescribers using Connect Care. 

Areas where key changes will occur include: Medication Management and Medication Reconciliation, Therapy Plans, and Transfusion Medicine. Note that some of these changes will not be covered in the "What's New" tool; details on these changes will be added to this page when available. 

There will also be a Connect Care-wide downtime the day of the upgrade (November 19, 00:30-04:30), plus a transparent lockdown prior to the downtime that will impact the creation of new personalizations (November 13-19). Watch the Connect Care Bytes Blog for details.
