Blood Restrictions Assessment
As of January 2026, the "Blood Restrictions Assessment" worksheet in the Consent & eForms Navigator provides a dedicated place in Connect Care to document blood restrictions, including specific blood components and/or products. This worksheet does not change workflows for informed consent process, but standardizes the ability to document blood restrictions and increase visibility in the system for patients with blood restrictions.
Documented blood restrictions flow into the applicable Consent eForms for Consent to Surgery or Invasive Procedure and Consent to Treatment Plan or Procedure (as a template).
If a patient has restrictions on which blood components and/or products they can receive but there is no upcoming treatment or procedure, the Advanced Expressed Wishes and Instructions – Blood Components and Products consent form can be used if other consent or advanced care planning documentation with specificity for blood restrictions does not exist.

FYI Flag
The Blood Restrictions FYI Flag (previously named "Blood Refusals") triggers automatically upon documenting blood restrictions.
When blood restrictions are updated, the user is prompted to review whether or not to remove the FYI Flag.

An OPA appears when a blood product/component (or related) is ordered, but the patient has documented blood restrictions.
